Pick-up Request Forms

For new customers:

Please make sure you have submitted a profile sheet to Remedy before you request a pick-up. If you have not done so already, please fill out and submit a Recyclable Materials Profile (pdf) or a Wastewater Profile (pdf).

After the profile is reviewed, processed, and approved you will receive a profile number. You can then proceed to Request a Pick-Up (pdf). Please insert the profile number in the pick-up request form.

For existing customers:

Please fill out the Pick-Up Request Form (pdf) and submit through e-mail.
